This play call from John Smith (yes, their coaches name is John Smith. The whitest name ever) was bad, but I’m not sure how high it ranks in worst trick plays of all time. It doesn’t compare to this:

During my quick search, I couldn’t find one that was necessarily worst than both the Colts one and the one ran by Oregon State. There’s A LOT of bad fake punts/kicks, but not that many bad trick plays.
